Dr. Youssef is a US Civil Surgeon and can conduct your full medical exam and other requirements that you will need.
Please bring a copy of your medical records to your appointment. This includes medical history and immunization records. All records must be translated into English and notarized.
Immigration exams are not covered by insurance.

Direct Primary Care (DPC) is an innovative healthcare model where physicians partner with their patients to provide primary care services under a flat, periodic membership fee. By moving outside of the administrative burdens of the traditional insurance-based billing system, DPC practices are able to provide a wide spectrum of care in a transparent, affordable manner. DPC practices—now a grassroots movement numbering over 1000 nationwide—are able to do this while offering patients a higher level of service, communication, and relationship, than what is often found in our dysfunctional healthcare system.
Philosophically, the model is intended to restore the patient-physician relationship without the intrusion of insurance companies.
Our Membership program will still allow a patient to have their insurance but we will not bill your insurance. Our transparent labs and imaging service pricing is available and your membership allows for discounts on labs and up to 10 medical visits per year as well as one wellness full physical visit per year.

Please bring your completed form (top portion filled out), along with translated and notarized medical and immunization records. Note that immigration exams are not covered by insurance.
