Do I need a primary care provider?

What insurances do you take? Do you also take self-pay?

What are your COVID-19 Guidelines?

We follow the CDC guidelines as it relates to COVID-19.  We require face covering while in the office, social distancing and hand hygiene which all must be strictly observed while in our office.

Does your clinic offer telemedicine visits?

Yes we can do visits online for acute concerns or follow up appointments. Wellness visits and return to work visits must be done in person.

What should I do to prepare for my first appointment?

You will be emailed patient registration forms which should be completed prior to your arrival.  Be sure to arrive early to give yourself plenty of time to complete the initial intake process.  You can also bring any relevant lab reports, medication and treatment plans you’re currently under to discuss.

What kinds of payment do you accept?

If you have a deductible or co-pay, payment is due at time of service. Your insurance benefits to confirm the payment required. We accept cash, check, debit or credit card.

What happens if I don't show up for my appointment?

We request that you cancel your appointment 48 hours prior to your scheduled time to allow someone else to take your slot.  In the event you do not cancel and fail to show up for an appointment, you will be charged your copay amount for the missed appointment.

Does your clinic have evening hours or walk in hours?

Walk-ins welcome during business hours. If you need an evening appointment please call ahead at (708) 581-4960 and we can try our best to accommodate your request.
